WHAT IS A D4 MEDICAL?
The D4 medical examination is a DVLA-required assessment for drivers of large goods vehicles (LGV/HGV) and passenger carrying vehicles (PCV). It confirms that you are medically fit to drive a commercial vehicle.
HOW OFTEN DO YOU NEED A D4 MEDICAL?
● First application for a Category C licence: D4 required at time of application
● Renewal from age 45: every 5 years
● Renewal from age 65: every year
WHAT DOES A D4 MEDICAL INCLUDE?
The examination covers:
● Vision assessment (visual acuity and field of vision)
● Blood pressure measurement
● Urine analysis
● Cardiovascular health check
● Assessment of any medical conditions that may affect fitness to drive (diabetes, epilepsy, sleep apnoea, etc.)
● General physical examination
HOW LONG DOES IT TAKE?
A standard D4 medical takes approximately 30-45 minutes.
WHY ARE NHS WAITS A PROBLEM FOR HGV DRIVERS?
Many HGV drivers struggle to get D4 medicals arranged through their NHS GP — either because their GP doesn't offer them, or the waiting time is too long and risks a gap in their licence. This can cost drivers their livelihood.

WHAT CONDITIONS MIGHT AFFECT YOUR HGV LICENCE?
