Respiratory illnesses — from the common cold to flu, COVID-19, chest infections, and pneumonia — remain among the most common reasons for sick leave in the UK. Here's when you need a note and how to get one fast.
THE 7-DAY RULE FOR RESPIRATORY ILLNESS
As with all illnesses, you can self-certify the first 7 calendar days of absence. After that, your employer can request a fit note.
Most mild respiratory illnesses (a cold, mild flu) resolve within 7 days, meaning many people won't need a note at all. But COVID-19 and more serious infections can keep people off work for much longer.
WHEN TO GET A FIT NOTE FOR RESPIRATORY ILLNESS
Consider getting a fit note if:
● Your illness has lasted more than 7 calendar days
● Your employer has asked for one (they may have a policy requiring notes earlier)
● Your illness is particularly severe — pneumonia, severe COVID, etc.
● You're at risk of complications and your doctor has advised rest
COVID-19 SICK NOTES
Since the end of the COVID-specific isolation rules, COVID-19 sick notes work exactly the same as any other illness. There is no longer a government-issued COVID certificate. If you need documentation of COVID absence, a standard fit note from a doctor covers this.
LONG COVID
Long COVID — persistent symptoms lasting weeks or months after the initial infection — can significantly affect the ability to work. Dr Sick can issue fit notes for Long COVID covering fatigue, breathlessness, cognitive difficulties ("brain fog"), and other symptoms.
