If you've never looked closely at a fit note before, you might not know exactly what information it needs to contain. Here's a complete breakdown of what makes a UK fit note legally valid.
THE OFFICIAL FIT NOTE FORMAT
The Statement of Fitness for Work (Med 3) form must include the following: Patient name — your full name as it appears on official documents.
Date of assessment — the date the doctor assessed you (which may be earlier than the date of issue if backdated).
Assessment outcome — one of two options: "Not fit for work" or "May be fit for work."
If "may be fit for work," the recommended adjustments — such as amended duties, altered hours, workplace adaptations, or a phased return.
Duration — how long the note applies for. Short-term notes typically cover 1-2 weeks; long-term notes can cover up to 3 months.
Clinical details — a description of the condition or its effects (it doesn't have to name a specific diagnosis if the patient prefers privacy).
Doctor's details — name, GMC registration number, and practice details.

WHAT ABOUT THE PAPER MED 3 FORM?

YOUR EMPLOYER'S RIGHTS
Your employer can request a fit note after 7 days of absence. They cannot demand one for shorter periods unless your contract specifies otherwise. They also cannot demand that the note come from your own GP specifically.
